Clinical Account Forms
Please view our guide to requesting clinical systems access which is available from the CITS self-service portal and covers the most common systems we provide access to.
The forms below are for use as a fail-safe or for colleagues who do not have access to our self-service portal e.g. they are based at GP sites or NHS Trusts outside Cornwall, or work for partner organisations.
- Aria: Account Request Form
- C-Cube: Application Form
- ICIP: ICIP Account Request Form for the ICIP CareVue system and Winpath Ward Enquiry.
- ICIP (SNOD): Account Request Form for the ICIP CareVue system.
- Insight External Trust:Account Request Form. This form should be completed only when a member of staff from Exeter, Torbay, North Devon and Plymouth Hospitals NHS Trust needs access to Royal Cornwall’s PACS images for the diagnosis and treatment of patients.
